Renewal Period	Every two years; Self Audits on non-audit year at $25/office.
Renewal Certification Fee	$2,295 Application and Renewal Certification Fee. $625 for additional office on-site evaluations (10% of total number of offices, not to exceed 15 locations). $25 per office/branch not receiving on-site audit, for review of Self Audit.
